Cook was raised in Cambridge, Massachusetts, in a Catholic family of Irish descent, alongside an older half-brother and five sisters. During his childhood, he was introverted and shy, but he began acting and performing stand-up comedy during his junior year of high school. After completing high school, Cook studied graphic design in college as a backup career plan. He subsequently utilized his design background to create his own promotional merchandise and album artwork.
Beginning his comedy career in 1990, Cook performed in local clubs before relocating to New York City in 1994 and Los Angeles in 1996. His main breakthrough arrived in 1998 following an appearance on Comedy Central's Premium Blend. In 2000, he starred in a Comedy Central Presents special, which twice won the network's Stand-up Showdown. He signed with Comedy Central Records to release his certified platinum debut album, Harmful If Swallowed, in 2003. His 2005 follow-up album, Retaliation, went double platinum and entered the top five on the Billboard chart.
Cook expanded his career through major live performances, television specials, and business ventures. In 2005, he founded Superfinger Entertainment to produce his own media projects. In November 2007, he became the second comedian to sell out the main arena space at Madison Square Garden, performing two shows in a single night. Cook also established endurance records at the Laugh Factory, capped by a seven-hour onstage performance in January 2008. He continued producing specials and touring, including his 2014 Showtime special Troublemaker and his 2019 Tell It Like It Is Tour.
In addition to stand-up, Cook established a film career starting in the late 1990s with appearances in Mystery Men and Waiting.... He went on to secure leading and supporting roles in movies such as Employee of the Month, Mr. Brooks, Good Luck Chuck, Dan in Real Life, My Best Friend's Girl, and 400 Days. On television, his early appearances included the 1995 comedy series Maybe This Time, alongside hosting engagements on Saturday Night Live. On stage, he portrayed Franz Liebkind in a 2012 production of The Producers at the Hollywood Bowl.
Cook's comedic style centers on high-energy, long-form storytelling and physical persona, inspired by the contrasting personalities of his mother and father. His professional achievements earned him Rolling Stone's Hot Comic of the Year, the Big Entertainer Award at the VH1 Big in '06 Awards, and the 2007 Teen Choice Award for Best Comedian.
