She was also an Olympic-trained ice skater, though a knee injury halted her athletic pursuits. She pursued performing arts throughout her education and later earned a degree from the University of Southern California's School of Cinema-Television, where she belonged to the Kappa Alpha Theta sorority. She subsequently relocated to New York City to perform in Shakespeare festivals and summer stock theater.

Alexander began her professional screen career with roles on short-lived television programs such as the medical series Presidio Med and the drama Wasteland. She gained widespread public attention when she portrayed Gretchen Witter in the fourth season of the drama Dawson's Creek. Around this time, she made guest appearances on series like Greg the Bunny, CSI: Crime Scene Investigation, and Friends, while also securing roles in feature films such as All Over the Guy and Lucky 13.

In 2003, Alexander was cast as NCIS Special Agent Caitlin "Kate" Todd on the CBS procedural series NCIS. She remained with the show for its first two seasons until her character was killed off in the May 2005 finale. While initial reports cited a demanding workload as the reason for her departure, Alexander later clarified in 2023 that she left to step away from a toxic workplace environment, alleging workplace bullying by a colleague. Previously filmed footage of her was later repurposed for a 2012 episode.

Following her exit from NCIS, Alexander appeared in feature films including Mission: Impossible III, the comedy Yes Man, He's Just Not That Into You, and the independent drama The Last Lullaby, as well as the television series The Nine. In 2010, she landed the starring role of Chief Medical Examiner Dr. Maura Isles on the TNT series Rizzoli & Isles, performing opposite Angie Harmon. She portrayed the character across seven seasons through 2016, earning recognition for providing positive visibility for autism spectrum disorder traits.

During her tenure on Rizzoli & Isles, Alexander secured a recurring role on Showtime's Shameless, playing university professor Helene Runyon across its fifth and sixth seasons. Her subsequent acting work includes the 2018 film The Ride, a guest role on Law & Order: Special Victims Unit, and a detective role in the 2020 film Dangerous Lies. She also lent her voice to the animated web series Deathstroke: Knights & Dragons in 2020 and expanded into directing with a 2021 episode of the Netflix series You.

In her personal life, Alexander's initial marriage to Luka Pecel ended in an annulment. In 2007, she married film director Edoardo Ponti, and the couple has a daughter named Lucia and a son named Leonardo. She is multilingual, speaking English, Serbian, Italian, and basic French. In late 2018, she publicly defended former co-star Michael Weatherly after workplace allegations were brought against him.
