Miller was born on June 2, 1972, in Chipping Norton, Oxfordshire, England, to American parents. His mother, a special education teacher, and his father, a lawyer and teacher studying at the University of Oxford on a Rhodes Scholarship at the time, moved the family to Brooklyn, New York, when Miller was a year old. His father is of African-American, Jamaican, German, and English descent, while his mother has Rusyn, Swedish, French, Dutch, Syrian, and Lebanese heritage. Miller attended Midwood High School in Brooklyn and graduated from Princeton University in 1995 with a BA in English. During his college years, he performed with the Princeton Tigertones a cappella group and was a member of the Quadrangle Club and Colonial Club.

In 1995, Miller relocated to Los Angeles to pursue an acting career, making his television debut on Buffy the Vampire Slayer in 1998. He landed his first starring role in the 2002 mini-series Dinotopia, followed by co-starring appearances in the 2003 feature films The Human Stain and Underworld. Around this period, he was also featured in two Mariah Carey music videos directed by Brett Ratner.

Miller achieved his career breakthrough in 2005 when he was cast as Michael Scofield in Fox's Prison Break, portraying a structural engineer executing a plan to break his brother out of death row. The role, which required extensive special effects application for his character's full upper-body tattoo, brought him critical recognition and a Golden Globe nomination. Miller played the character across the show's original four-season run through 2009 and reprised the role for a fifth season in 2017.

Alongside his acting work, Miller launched a screenwriting career under the pseudonym Ted Foulke. His debut screenplay for the 2013 film Stoker was featured on the 2010 Black List of unproduced screenplays. He later wrote the 2016 horror film The Disappointments Room and engaged in negotiations to adapt the novel The Story of Edgar Sawtelle. In front of the camera, he portrayed Chris Redfield in Resident Evil: Afterlife, guest-starred on Law & Order: Special Victims Unit, and was cast in 2014 as Leonard Snart / Captain Cold on The Flash before becoming a main regular on Legends of Tomorrow.

Miller resides in Vancouver, British Columbia, holding American citizenship. He publicly came out as gay in August 2013 through a letter published by GLAAD declining an invitation to a Russian film festival due to the country's anti-LGBT legislation. He has publicly discussed his struggles with childhood depression, past suicidal ideation, and his work as an ambassador for the mental health charity Active Minds. In July 2021, Miller shared that he had been diagnosed with autism.

In 2020, Miller announced he was no longer interested in playing straight characters, stepping away from future iterations of Prison Break. He subsequently narrated the 2023 audiobook release of Prayers for Bobby. In 2024, reports announced that he would reunite with his Prison Break co-star Dominic Purcell for the drama series Snatchback.
