Shaw was born on July 10, 1958, in Farranree, County Cork, Ireland, to a physicist mother and an ophthalmic surgeon father. Raised alongside three brothers in Cork, she attended Scoil Mhuire and earned a degree in philosophy from University College Cork. She subsequently trained at the Royal Academy of Dramatic Art in London, graduating in 1982. Upon entering the actors' union Equity, she adopted her grandmother's maiden name, Shaw, to differentiate herself from another member and to pay tribute to playwright George Bernard Shaw.

Shaw's theatrical career began to flourish in the 1980s through performances in productions such as The Rivals, As You Like It, and Les Liaisons Dangereuses. She built a long-standing creative relationship with director Deborah Warner, collaborating on major classical and modern works. Shaw took on notable stage roles, including the lead in Electra, The Good Person of Szechwan, Hedda Gabler, and playing the title male character in Richard II. Her stage work earned her Laurence Olivier Awards for Best Actress in 1990 and 1994, alongside multiple additional nominations.

In 1996, Shaw presented a critically acclaimed solo performance of T. S. Eliot's poem The Waste Land in New York, winning a Drama Desk Award. She later debuted on Broadway in the title role of Medea, receiving a Tony Award nomination. Her broad stage work also includes starring roles in Mother Courage and Her Children, John Gabriel Borkman, and The Testament of Mary, the latter of which she performed on Broadway and for which she received a United Solo Special Award.

In cinema, Shaw achieved broad international recognition playing Petunia Dursley across five entries of the Harry Potter film series from 2001 to 2010. Beyond this franchise, her notable film appearances include roles in My Left Foot, Persuasion, Jane Eyre, The Tree of Life, Colette, Ammonite, and Enola Holmes. She also held roles in feature films such as Super Mario Bros., The Butcher Boy, and The Black Dahlia.

Shaw has also sustained an active television career, appearing in projects such as RKO 281 and True Blood. She gained significant acclaim for portraying MI6 official Carolyn Martens in the series Killing Eve, earning a BAFTA TV Award for Best Supporting Actress along with Emmy nominations. Her television presence expanded with nominated roles in Fleabag, as well as major parts in Baptiste, the Star Wars series Andor, True Detective, and Bad Sisters.
