He first gained widespread popularity through television series in the late 1990s before shifting his career focus exclusively to feature films. Over his career, he has achieved prominent recognition as a leading actor, film executive, and industry advocate.

Raised in Hong Kong, Koo attended St. Teresa's School Kowloon, Carmel Secondary School, and Carmel Divine Grace Foundation Secondary School. His father was an actor who later became a businessman. In November 1990, Koo was convicted for involvement in a robbery alongside friends and served 22 months at the Correctional Services Lai King Training Centre. Between 1992 and 1993, he worked as a modeling agent and appeared as a model in advertisements and karaoke videos.

Koo signed with TVB in 1993 and broke out in 1995 starring in the wuxia drama The Condor Heroes 95. He adopted an extreme tan in 1997 to expand into contemporary roles. At TVB, he earned Most Popular Actor Awards for Detective Investigation Files IV in 1999 and A Step into the Past in 2001, as well as a Favorite Character Award for At the Threshold of an Era II in 2000. The popularity of these broadcasts built him a large fan base across Hong Kong, Taiwan, mainland China, and Southeast Asia.

Transitioning fully to cinema after 2001, Koo signed with China Star Entertainment Group and starred in romantic comedies such as La Brassiere and Naked Ambition. He launched a major collaboration with director Johnnie To and Milkyway Image starting with Throw Down in 2004, followed by acclaimed roles in Election 2, Romancing in Thin Air, and Drug War. His diverse filmography includes Protégé, Run Papa Run, Connected, Overheard, and The White Storm. For his lead performance in Paradox (2017), he won Best Actor awards at the Hong Kong Film Awards, Asian Film Awards, and Hong Kong Film Directors' Guild Awards.

In 2013, Koo founded One Cool Group Limited, acquiring visual effects and prosthetic make-up companies to advance sci-fi and local cinema. He produced and starred in Warriors of Future, initiated a film adaptation of A Step into the Past, and partnered with Media Asia to form Cool Style in 2021. He also served as executive producer on Hollywood animated films such as The Mitchells vs. the Machines, Vivo, and The Garfield Movie.

Beyond film, Koo has authored books, released music albums, designed eyewear, and endorsed numerous commercial brands. He served as ambassador for the Hong Kong International Film Festival from 2014 to 2018 and was elected president of the Hong Kong Performing Artistes Guild in 2018. Following the 2008 Sichuan earthquake, he quietly established a charity foundation that financed dozens of schools, clinics, and water projects in rural China. He has also served as a UNICEF ambassador since 2007.
