Macfarlane was born in London, Ontario, where his father Thomas was the director of student health services at the University of Western Ontario and his mother Penny worked as a mental health nurse at a London hospital. He grew up alongside his twin sister, Ruth, and an older sister, Rebecca. He attended the Lester B. Pearson School for the Arts and London Central Secondary School before moving to New York City to study drama at Juilliard.

Early in his screen career, Macfarlane appeared opposite Cynthia Nixon in Robert Altman's miniseries Tanner on Tanner on the Sundance Channel. He subsequently secured leading roles in the 2005 FX series Over There, playing PV2 Frank "Dim" Dumphy, and in the 2009 two-part miniseries Iron Road. His major breakthrough came when he was cast as Scotty Wandell on Brothers & Sisters, playing the husband of Kevin Walker, portrayed by Matthew Rhys.

Macfarlane went on to star in various other television productions, including playing Jason Howell in the Canadian sitcom Satisfaction, Rick Lincoln on NBC's The Night Shift, Chaplain Hopkins on PBS's Mercy Street, and D'avin Jaqobis in Syfy's Killjoys. He also guest-starred as Sam Parker on an episode of Shifting Gears on January 28, 2026. Beginning in 2014, Macfarlane became a frequent romantic lead in Hallmark Channel original movies, including titles such as The Memory Book, Christmas Land, Maggie's Miracle Christmas, The Birthday Wish, The Mistletoe Promise, and Chateau Christmas.

In feature films, Macfarlane co-starred with Billy Eichner in the 2022 movie Bros, marked as the first gay romantic comedy from a major studio featuring an openly LGBTQ+ principal cast. Earlier in his creative career, Macfarlane served as the lead singer and songwriter for the band Fellow Nameless, originally formed as Slipnaught during his eighth-grade year. The band recorded an underground CD album and ten additional unreleased songs before the remaining members eventually reorganized under new band names without him.

Macfarlane has also maintained a stage career. He appeared in Off-Broadway plays such as Juvenilia in 2003 and the American premiere of Where Do We Live in 2004 at the Vineyard Theatre, which was cited by the 2005 GLAAD Media Awards. He later starred in The Busy World is Hushed at Playwrights Horizons in 2006, reprising his role for its Los Angeles premiere in 2007. He joined a celebrity staging of Howard Ashman's unproduced musical Dreamstuff and starred in the world premiere of the stage drama Reverberation at Hartford Stage in February 2015.

Macfarlane came out as gay in an April 2008 interview with The Globe and Mail and was naturalized as an American citizen on June 12, 2018. Outside of acting, he plays the cello—which was featured in the movie Chateau Christmas—as well as the trumpet. On June 4, 2023, Macfarlane and his partner, alpine skier Hig Roberts, announced the birth of their first child.
