Raised alongside an older brother, he completed his secondary education at Youngdong High School before attending Chung-Ang University, where he earned a bachelor's degree in Theater Studies in 2004 and subsequently obtained a master's degree in 2009.

Although his planned feature debut in 2002 was halted due to financial constraints, Hyun formally debuted in the 2003 television series Bodyguard. He subsequently appeared in the sitcom Nonstop 4, the drama Ireland, and the sports movie Spin Kick. His breakthrough arrived in 2005 with the romantic comedy My Lovely Sam Soon. The series achieved massive popularity, solidifying his status as a prominent figure across South Korea and Asia while earning him top honors at the MBC Drama Awards.

Following lead roles in the film A Millionaire's First Love and the television drama The Snow Queen, Hyun expanded his repertoire with diverse projects, including the film I Am Happy and the series Worlds Within and Friend, Our Legend. In 2010, he starred in the fantasy romance series Secret Garden. The show became an enormous cultural success, and his performance earned him the Grand Prize for Television at the 47th Baeksang Arts Awards, as well as the title of Gallup Korea's Television Actor of the Year in 2011. That same year, he featured in the films Come Rain, Come Shine and Late Autumn, both of which were showcased at the 61st Berlin International Film Festival.

In March 2011, Hyun enlisted in the South Korean Marine Corps for mandatory military service. Recognized as a top marksman during training, he served on active duty near the front line on Baengnyeongdo. Upon completing his service in December 2012, he was honored with the Secretary of Defense Award and the Marine Corps Commandant Commendation. Shortly after his discharge, he co-founded the agency O& Entertainment, and later established VAST Entertainment in 2016.

Hyun resumed his screen career with the 2014 historical film The Fatal Encounter, followed by the drama series Hyde Jekyll, Me in 2015. He enjoyed further success with the action thriller Confidential Assignment in 2017 and its 2022 sequel, along with the crime film The Swindlers. He also starred in the 2018 film The Negotiation, the zombie movie Rampant, and the drama series Memories of the Alhambra. Between 2019 and 2020, he starred in the romantic series Crash Landing on You, which received wide international acclaim and earned him the Grand Prize at the APAN Star Awards. His later achievements include starring in the 2023 film The Point Men and winning Best Actor at the Blue Dragon Film Awards in 2025.

Beyond acting, Hyun has engaged in philanthropic initiatives, supporting organizations such as the Community Chest of Korea, Save the Children, and the Hope Bridge Disaster Relief Association. He was also honored with the President's Award on Savings Day in 2013 for his personal financial management. In March 2022, he married actress Son Ye-jin, and the couple welcomed a son later that year.
