Falco was born on July 5, 1963, in Brooklyn, New York, to Judith Anderson, an actress, and Frank Falco, a jazz drummer and advertising worker. Raised primarily on Long Island, she grew up in Hicksville, North Babylon, West Islip, and Northport. She acted in theater productions as a youth before graduating from Northport High School in 1981. Falco earned a Bachelor of Fine Arts in acting from the State University of New York at Purchase in 1986.
She made her feature film debut in Sweet Lorraine (1987) and gained attention for her lead performance in the 1992 independent film Laws of Gravity. Her early television career included appearances on Law & Order, Homicide: Life on the Street, and the HBO prison drama Oz. In 1999, she made her Broadway debut in the play Side Man and starred in the film Judy Berlin.

She later appeared off-Broadway in The Madrid in 2013 and in the web series Horace and Pete in 2016.
In subsequent years, Falco starred as attorney Leslie Abramson in Law & Order True Crime: The Menendez Murders (2018), earning another Emmy nomination. Her later work includes lead roles in the police drama Tommy (2020), portraying Hillary Rodham Clinton in Impeachment: American Crime Story (2021), and playing Amy Davidson in the series Bupkis (2023). On film, her appearances include Avatar: The Way of Water (2022) and Avatar: Fire and Ash (2025).
In her personal life, Falco adopted a son in 2005 and a daughter in 2008. She became sober in the early 1990s and was diagnosed with breast cancer in 2003. A vegan, Falco is active in animal advocacy work with organizations such as PETA.
