Born and raised in Tokyo on November 2, 1982, Fukada entered the entertainment industry as a teenager. In October 1996, at the age of thirteen, she earned top honors at the 21st Talent Scout Caravan Grand Prix during the Pure Girl Audition. She later balanced her early professional commitments with her education, graduating from Horikoshi High School in 2001.

Fukada launched her acting career in 1997 with a role as Kazune Mizuno in the television drama Sore ga Kotae da!. Her film debut followed in 1999 when she played Kanae Sawaguchi in Ring 2. Over the next few years, she expanded her feature film credits with appearances in School Day of the Dead in 2000 and the Takeshi Kitano film Dolls in 2002, where she portrayed the character Haruna. She achieved critical recognition for her performance in Kamikaze Girls, winning the Best Actress award at the Yokohama Film Festival.

Alongside her screen work, Fukada established a musical career as a J-pop singer signed with Pony Canyon. She debuted her recording career in 1999 with the release of her single The Last Fruit and her initial album, Dear…. Decades after their original debut, her musical catalog was made available worldwide on digital music stores and streaming platforms in July 2023.

In May 2021, Fukada put her career on hold following a diagnosis of adjustment disorder, which had been preceded by period of ill health beginning in early 2020. Prioritizing medical treatment, her agency announced an temporary hiatus, leading her to withdraw from her scheduled lead role in the July 2021 television series My Oshi no Oujisama. She completed her medical leave and officially resumed her professional engagements in September 2021.

In her personal life, Fukada was publicly linked to high-profile businessman Hiroyuki Sugimoto beginning in 2018. Following widespread public interest regarding potential marriage plans, the couple's separation was confirmed in 2023.
