She studied acting at Santa Ana College and Orange Coast College before dropping out after a year to move to Manhattan. Upon joining the Actors' Equity Association, she adopted her mother's maiden name, Keaton, because another actress was registered as Diane Hall. She then studied acting at the Neighborhood Playhouse using the Meisner technique.
Keaton began her professional career on stage in 1968 as an understudy in the original Broadway production of the musical Hair. The following year, she starred in Woody Allen's play Play It Again, Sam, earning a Tony Award nomination for Best Featured Actress in a Play. She made her film debut in Lovers and Other Strangers (1970) before securing her breakthrough role as Kay Adams in Francis Ford Coppola's The Godfather (1972). She later reprised the role in The Godfather Part II (1974) and The Godfather Part III (1990).
Throughout the 1970s, Keaton established herself as a leading film actress through collaborations with Allen, starring in comedies such as Sleeper (1973), Love and Death (1975), and Annie Hall (1977). Her title performance in Annie Hall won her the Academy Award for Best Actress. The role also established her as a major fashion icon due to her character's distinctive wardrobe of vintage men's clothing. During this period, she also took on dramatic roles in films including Looking for Mr. Goodbar (1977) and Interiors (1978).
Keaton received three additional Academy Award nominations for Best Actress for her performances in the historical drama Reds (1981), the family drama Marvin's Room (1996), and the romantic comedy Something's Gotta Give (2003). Her extensive filmography includes roles in Manhattan (1979), Shoot the Moon (1982), Baby Boom (1987), Father of the Bride (1991) and its sequel, The First Wives Club (1996), Finding Dory (2016), and Book Club (2018). On television, she earned award nominations playing Amelia Earhart in Amelia Earhart: The Final Flight (1994) and later starred in the series The Young Pope (2016). As a filmmaker, she directed three narrative feature films and the documentary Heaven (1987).
In addition to acting, Keaton pursued still photography, publishing a collection of hotel interior photographs titled Reservations in 1980. She also authored four books, including her 2011 memoir Then Again. Known for her distinct personal fashion style, she frequently favored conservative attire, turtlenecks, gloves, and vintage men's clothing.
