Ward was born in Perth, Western Australia, to Gary Ward, an Australian doctor, and Claire, an English nurse. Raised alongside three siblings, including an older sister who also became a model, Ward attended Presbyterian Ladies' College and Shenton College. Her interest in acting developed at age ten when she played a witch in a school production of Hansel and Gretel. In 2002, at age fourteen, she was accidentally scouted at the Search for a Supermodel competition, which she had attended to support friends. Though she did not win, she was signed by Vivien's Model Agency and scouted by IMG's David Cunningham.
Ward made her Australian Fashion Week debut in May 2003 at age 15. Her baby doll-like look inspired a new generation of models. In September 2004, at age sixteen, she became the youngest model on the cover of American Vogue as one of nine Models of the Moment. By the end of 2004, she had appeared on covers for Australian, British, French, and Japanese editions of Vogue. In subsequent years, Ward walked numerous New York and European runways, starring in campaigns for brands including Calvin Klein, Prada, Burberry, Valentino, Dolce & Gabbana, Balenciaga, Hermès, Dior, and Givenchy. She appeared on inaugural covers for Vogue China in 2005 and Vogue India in 2007, and was declared one of the top 30 models of the 2000s by Vogue Paris.
Following the spring 2008 runway shows, Ward took an extended break from modelling to focus on acting and personal growth, a hiatus spurred by the death of Heath Ledger. During her break, she studied at the Stella Adler Studio of Acting in New York and made her stage debut in 2011 with the Perth Theatre Company in The Ugly One. After signing with IMG Models Australia in 2013, Ward returned to the runway in September 2014 at the Prada Spring/Summer 2015 show during Milan Fashion Week.
Following her return, Ward appeared on several Australian Vogue covers, fronted campaigns for Prada, Country Road, and David Webb, and walked for designers such as Ellery, Proenza Schouler, Alexander McQueen, and Calvin Klein menswear. She also featured in editorials and covers for publications including Elle Australia, Russh, Allure, Vogue Italia, Vogue Thailand, Australian Harper's Bazaar, and Numéro Tokyo, and served as a guest judge on Australia's Next Top Model.
Ward's film career began with the 2008 Australian feature The Black Balloon, followed by roles in the thriller The Strangers (2008) and Pirates of the Caribbean: On Stranger Tides (2011). In her personal life, Ward dated actor Heath Ledger until his death in January 2008. She and her partner David Letts have three children, born in December 2013, January 2017, and June 2020.
