McIntosh is the daughter of Greg McIntosh and former Canadian Olympic swimmer Jill Horstead. During early childhood, McIntosh and her older sister, Brooke McIntosh, both competed in figure skating and swimming. While her sister became a competitive pair skater, McIntosh dropped figure skating to focus on swimming. As a junior competitor, she broke over 50 Canadian national Age Group Records. In May 2021, at age 14, she swam a 4:05.13 in the 400 meter freestyle, the fastest time recorded worldwide by a 14-year-old female swimmer.
McIntosh gained international recognition after qualifying for the 2020 Summer Olympics in Tokyo as the youngest member of the Canadian team. At the Games, she placed fourth in the 400 meter freestyle with a Canadian record time of 4:02.42 and finished fourth in the 4 × 200 meter relay. Later in 2021, at the World Swimming Championships in Abu Dhabi, she won a gold medal in the 4 × 200 metre freestyle relay, a silver in the 400 metre freestyle, and a silver in the 4 × 100 metre medley relay.
In 2022, McIntosh made her long course World Aquatics Championships debut in Budapest, winning gold medals in the 200 metre butterfly and 400 metre individual medley, silver in the 400 metre freestyle, and bronze in the 4 × 200 metre freestyle relay. She became the youngest Canadian world champion in history and the first Canadian to win two golds at a single World Championships. Later that year at the Commonwealth Games in Birmingham, she won gold in the 200 metre and 400 metre individual medleys, alongside three silver medals and one bronze medal in freestyle and relay events.
In March and April 2023, McIntosh set her first two world records in the 400 meter freestyle and 400 meter individual medley within five days at the Canadian national trials, becoming the first swimmer to hold both long course records simultaneously. At the 2023 World Aquatics Championships in Fukuoka, she successfully defended her title in the 200 metre butterfly and earned a bronze medal in the 200 metre freestyle.
At the 2024 Summer Olympics, McIntosh won four individual medals, consisting of three gold medals and one silver medal. Her honors include the Northern Star Award as the top Canadian athlete of 2024, three consecutive Bobbie Rosenfeld Awards as Canadian female athlete of the year from 2023 to 2025, and two World Aquatics Female Swimmer of the Year awards for 2024 and 2025.
