Berger was born during the Second World War to musician Josef Berger and teacher Therese Jany. She first appeared on stage at age four, singing while accompanied by her father on the piano, and began ballet lessons at age five. After taking private acting lessons, she landed a small first role in the 1957 film The Unexcused Hour. She was accepted into the Max Reinhardt Seminar acting school in Vienna, but left shortly afterwards after taking a film role without permission. In 1958, she became the youngest member of the Josefstadt Theatre in Vienna, appearing in productions such as L'Œuf, Charley's Aunt, Much Ado About Nothing, and Cat on a Hot Tin Roof.

In 1960, Berger appeared in the comedy film The Good Soldier Schweik. Moving to Hollywood in 1962, she worked alongside prominent actors including Charlton Heston, Dean Martin, Frank Sinatra, Richard Widmark, John Wayne, Kirk Douglas, and Yul Brynner. Her screen work in the 1960s included the television episode The Double Affair for The Man from U.N.C.L.E. (later expanded into the feature film The Spy with My Face), Sam Peckinpah's Westerns The Glory Guys and Major Dundee, and feature films such as Cast a Giant Shadow, Our Man in Marrakesh, and The Quiller Memorandum. She also appeared in the pilot and a subsequent episode of the television series It Takes a Thief.

In 1965, Berger co-founded a film production company with Michael Verhoeven, whom she married in 1966. She expanded her European career in France and Italy, starring in 1970 in the first film produced by her company and directed by her husband. Her joint production company went on to make internationally successful films such as Die weiße Rose, The Nasty Girl, and My Mother's Courage. Alongside her film work, Berger returned to the stage, performing at the Burgtheater in Vienna, the Thalia Theater in Hamburg, and the Schiller Theater in Berlin. Between 1974 and 1982, she played Buhlschaft in Jedermann at the Salzburg Festival, and in 1977 she acted in the war film Cross of Iron. She also served on the jury of the Berlin International Film Festival in 1977 and 1998.

Berger achieved renewed television success in the mid-1980s with the serial Kir Royal, followed by Die schnelle Gerdi. In 1989, she began a singing career. Outside of acting, Berger participated in the 1971 media campaign "We've had abortions!" and campaigned for the Social Democratic Party in 1972. She published her autobiography in 2006. Berger was married to Michael Verhoeven until his death in 2024, and the couple has two sons, Simon and Luca.
